Schedule a routine or Urgent Care appointment for you. Urgent Care services are located in many of our medical centers, and 6 are open 24/7, along with lab, X-ray, and pharmacy services. You are covered for care at any Kaiser Permanente location in another. Kaiser Permanente Senior Advantage Basic Kern (HMO) formulary is divided into tiers or levels of coverage based on usage and according to the medication costs. Each tier will have a defined copay that you must pay to receive the drug. Drugs in lower tiers will usually cost less than those in higher tiers. Kaiser Permanente does not utilize SSN’s as Member ID numbers. NAME Name of the enrolled Member. Copay Primary & Specialty Care Visit copay 4. Urgent Urgent Care copay 5. Emergency Emergency Room visit copay 6. Group # Employer / Purchaser Group number 7. Effective Date Month, day and year the Member’s benefit period began. Filling an urgent care prescription. If you’re traveling and receive urgent care at a Kaiser Permanente facility, a military hospital or clinic, a TRICARE network provider and need medication, you can have the prescription filled where you get care or at the nearest TRICARE retail network pharmacy. Members who need urgent care services can visit a CVS MinuteClinic in 28 states where Kaiser Permanente doesn’t operate. Members pay only their standard copay or coinsurance at the time of service. After the visit, members will be charged for any additional cost.

Prescription Drugs
Kaiser members are automatically enrolled in the two-tier Kaiser prescription drug plan. Prescriptions are included in the annual Out-of-Pocket Maximum.
Manage your prescriptions online at kp.org or via the KP mobile app then pick-up prescriptions at a Kaiser pharmacy or enroll in the mail order program and save even more.

However, they would need to wait until they visit home and schedule their routine care appointments. - How to choose your Primary Care Doctor: Remember, you should have your annual physical with your Primary Care Doctor once every year. Visit kp.org/searchdoctors.
- Enroll at KP.org’s MyHealth Manager: Once enrolled you can email your doctor’s office with non-urgent health questions, schedule or cancel routine appointments, view most lab test results, refill prescriptions and have most mailed to your home, and find Kaiser locations and contact information on the go.
